2020 Basketball Registration and Team Roster

Please fill out all team, coach and athlete information completely. This form should be filled out by the team's coach.
  • Due to the inconsistency of determining pools by grades, the division of the team will be determined by the age of the oldest player. Age of athlete is determined by August 31, 2020. Teams may be asked to verify ages of athletes, so please bring a birth certificate of each athlete.
  • Please list all athletes that may play at some time during the tournament. Once the roster is submitted and the tournament begins, no changes or additions may be made.
